WHAT TO BRING
Soccer shoes (cleated and indoor), shin guards, rain gear, sun screen. Do not bring: valuables, jewelry or anything not clearly marked with your name.  Please bring a soccer ball (balls may be purchased for $25), water and a large water bottle, lunch and a snack. EQUIPMENT PROVIDED: One t-shirt is issued to every camper and balls are available for purchase for $25. 

MEDICAL CARE
A full-time athletic trainer will be on duty to administer to minor injuries and ailments. For more serious problems, campers will be transported to the local area hospitals. Written authorization to administer prescribed medications must be signed by the parent or guardian. This medication must be kept and administered by the athletic trainer.

MEDICAL INSURANCE & PHYSICAL EXAM
Proof of medical insurance, a copy of a physical exam within the past 12 months and a copy of immunization record's are required. Pursuant to state law, No camper will be allowed to participate until all forms are recieved. Inaddition, space is not secured in camp until all documents are received.

TUITION AND ENROLLMENT:
Please make checks payable to Kerr Camp, INC. Your canceled check will be your NOTIFICATION of your reserved spot in camp.   If an email address is provided, you will receive an emailed confirmation at least one week before camp begins. Send your completed application to KERR CAMP, INC., P.O. Box 920266, Needham, Ma 02492. There is a $25 fee for returned checks.

REFUND POLICIES
Campers sent home by the trainer due to an injury or illness will receive a pro-rated refund. If a camper is dismissed from camp, departs camp early, or arrives late for any reason other than specified above, full payment is required and refunds will not be processed. Cancellations made 3 or more weeks prior to beginning of the session, Kerrcamp will refund all fees less than the $50 service fee. Refunds will not be granted with less than 3 weeks notice.

MASSACHUSETTS DEPARTMENT OF PUBLIC HEALTH
This camp must comply with regulations of the Massachusetts Department of Public Health (105 CMR 430.000) & be licensed by the Concord Board of Health. Information on 105 CMR 430.000 can be obtained by calling the MDPH at 978-318-3275.
